Abstract: Soluble thrombomodulin (sTM) is a novel marker of damaged endothelial cell and acts as an anticoagulant. Plasma and urinary sTM in patients with minimal change nephrotic syndrome (MCNS) in children are demonstrated in this report. In spite of normal blood creatinine levels, the plasma concentrations of sTM in cases of onset and relapse of MCNS were elevated as compared with healthy subjects {at onset 37.8±14.9 TU/ml (Mean±SD), at relapse 21.8±11.1 TU/ml, normal 9.0±2.8 TU/ml}. The urinary concen- trations of sTM, however, were almost within normal range, and there was no correlation between plasma and urinary concentrations of sTM. Plasma concentrations of sTM had a good correlation (r=-0.742) with serum albumin. From these results it is suggested that high plasma sTM concentration in patients with MCNS at initial episode and in the course of relapse is likely to act toward anticoagulation.
