産褥期の母親のうつ症状の予防に向けた祖母の支援の検討

Abstract

本研究の目的は、産褥期の母親のうつ症状の予防に向けた祖母の支援方法を検討することである。1か月健診に来院した母親とその祖母に、産後の精神状態の知識や、その情報源、育児支援状況などについて質問紙調査を実施した。産褥期の母親の精神状態と、祖母の支援との関連をみた結果、祖母から支援を受けているEPDS陰性群の母親は、EPDS陽性群の母親に比べて、母性意識尺度における積極的・肯定的意識が有意に高かった。同じく、自己肯定感も有意に高いことが明らかとなった。また、「マタニティ・ブルーズは産後うつ病に移行することがある」という知識を持つ祖母は、抑うつ状態予防のために必要な支援である「母親の睡眠時間を確保する」支援を高い割合で行っていた。今回の結果から祖母のかかわりは産褥期の母親にとって、うつ症状の予防に向けた重要な支援者であると考えられる。さらに、祖母が産褥期の母親の精神状態についての知識を持つことは、産褥期のうつ症状の予防につながると考える。医療職者は、入院中の母親の様子や母親が求めている支援について祖母に情報提供を行うことが必要である。

Purpose: This study was conducted to examine the methods of support adopted by grandmothers to prevent depressive symptoms in puerperal mothers. Methods: Questionnaires were distributed to mothers 1 month after giving birth and grandmothers. The questionnaire asked grandmothers about their knowledge of the postpartum mental state of mothers and postpartum support, whereas mothers were asked about the support they wished to receive from grandmothers. The current menta1 state of mothers and their awareness about motherhood were also evaluated using the Edinburgh Postnatal Depression Scale (EPDS). Results: Grandmothers with knowledge of mothers' postpartum mental state were significantly more active in providing support to ensure time for the mother to sleep. Mothers in the EPDS-negative group received the support they desired from grandmothers. Furthermore, mothers in the EPDS-negative group had significantly higher awareness and self-esteem than those in the EPDS-negative group. Discussion: The results of this study showed that it is important for grandmothers to have knowledge about the mother's postpartum mental state. Nurses must communicate the condition of mothers to grandmothers during hospitalization and provide information regarding the support that mothers require.
