奈良県における在宅看護の課題と展望

Abstract

団塊の世代すべてが後期高齢者(75歳以上)となる2025年を視野に、地域包括ケアシステムを構築することを目指した取組みが全国で進められている。このような社会情勢の変化の中、在宅看護は、「地域完結型」の医療への変革、「在宅看取り」の体制整備、「自助」「互助」「共助」「公助」の機能強化等の課題解決に向けて多様な役割を担うことが期待されていると考えられる。そして看護職者は、地域包括ケアシステム構築に向けて、対象とする地域に応じて多層的に重なりあう役割を持っと捉え、各々が自己の立ち位置を確認し、様々な課題に向き合い、ポジティブなチャレンジで、役割を担っていくことが求められている。

Initiatives are being put forward nationwide with the aim of constructing an integrated community care system as we approach 2025 when the entire baby boomer generation will be in the elderly stages of life (75 years old or older). Amidst these changes in social conditions, home care nursing is anticipated to provide solution for important issues, such as complete regional type medical treatment, creating a system for end of life care and strengthening functions of self-support, mutual-support, communal-support and public-support. In preparation for the construction of an integrated community care system the nursing profession has to develop a multi-layered system with overlapping regional unit, and to define their individual tasks, face the problems head on and take on this challenging role with a positive attitude.
